Deal Feedback form: Ms. Kim would like to encourage more direct communication between parents and school staff members. Thus, in this collaboration, we will be able to distinguish wishes and suggestions from parents and improve the teaching process. In that regard, the Deal Feedback Form has been developed for that purpose. It will be included in each First Day Packet, but blank forms are also available in the office. The forms can be written to any member of the administrative staff (general matters) or to teachers for specific class related matters. School Health Form:Down load the health form here, although the nurse prefers the original form they send from school.

School Dental Form: Same as above

Field Trip Permission Slip: Teachers will usually send the field trip home completed for the specific trip, but if it is lost, you can use this one.

Deal Book Log Form: Your child needs to complete these forms for summer reading. This is a pdf of the booklist as well.
